By utilizing services under the FTP program, I have volunteered to participate in this program provided by SQUATS FITNESS PVT LTD ("SQUATS") and INSTITUTE OF NUTRITION AND FITNESS SCIENCES PVT LTD ("INFS") which may include, but may not be limited to, dietary guidance and exercise advise. I understand that the guidance provided to me is not by Certified Consultants but from students who are undergoing certification assessment at INFS. In lieu of my participation in this program, I agree to share my progress pictures which can be used by SQUATS and INFS on their website and social media for promoting their services and programs. In consideration of FTP Program agreement to instruct and train me, I do here now and forever release and discharge and hereby hold harmless FTP Program, it’s parent company SQUATS AND INFS and their respective agents, heirs, assigns, contractors, and employees from any and all claims, demands, damages, rights of action or causes of action, present or future, arising out of or connected with my participation in this or any exercise program including any complications or injuries resulting there from.

*You failed to complete the 4 weeks with the scholar.
*You post-poned the plan.
*You do not fill the feedback form.
*If you were enrolled under a mentor and still applied for FTP.
*You have availed the FTP services before.
